Identify any barriers to fully accessing what is important to you and for you.

Recommendations

We recommened specific service forms and codes to develop plans based on assessed needs.

Follow-up & planning

After assessment, we work with you to begin services and implement your support plan.

Independent Living Services (ILS)

Training and support for individuals to acquire and maintain skills to live independently.

Who It's For

Adults who want to live independently with up to 40 hours of support per month.

How We Help:
  • Service Planning: Create an individualized service plan based on the assessment.
  • Direct Services: Pair each client with a staff person to implement their plan.
  • Review and Support: Review goals at least semi-annually and update as needed.

Supported Living Services (SLS)

Supported Living Services assist adults with intellectual/developmental disabilities establish and maintain a safe, stable, and independent life in homes they own or rent. SLS helps individuals make meaningful choices toward their personal goals in relationships and the community. Services are offered for as long as needed and are flexible to a person’s changing needs over time.

Adults with developmental disabilities who want to live in their own home with ongoing support that adapts to their needs.
